雙手是一輩子累積的模樣，手指截肢了要保守不「保手」還是接回？

機台上突如其來的意外，A 先生被機器絞斷了右手食指，在醫院，A 先生面臨了困難的抉擇：接受斷指重建、以顯微手術的方式接回，不但在手術台上要好幾個小時，後續還有漫長的手指觀察期、再手術及復健，無法立即回到工作，一家人的經濟依靠頓失依據。

另外一個選擇比較簡單，醫師把斷端修整一下維持截肢的樣子，其他保守治療，病人隨即出院，安排復健，甚至有的人選擇不復健就回到工作崗位。

「醫生，我這個保守治療比較好？還是開刀比較好？」

不開刀的理由

保守治療 (conservative treatment)，並不等於不治療 (not no treatment) 而是如何維持醫生和病人同在治療一艘船上的學問 (keep patient on board)，身為外科醫師我們應該不只會手術適應症與禁忌症 (indication and contraindication)，應該還要會找個理由讓我不開刀 (find reason not to operate)。

我的思考是，立即手術處理傷肢 (immediate fixation) 在民眾心裡某種意義上也是立即從創傷當下很糟感覺中脫離的釋然 (means less suffer for patient)，加上若要保守治療，如何保護就變得很重要 (protection becomes an important issue)。

在保護患部觀念仍不完善的台灣、各式護木輔具並不普及價格亦無法貼近經濟條件較低的民眾，若要 conservative treatment，可能還需要更多數值 (parameter)、甚至病人在家自己可以用普及化的工具 (直尺、圓規) 或智慧型手機 app 測量、照相追蹤，對於各項變化 (pay attention to parameter changing) 要更敏感才行。 

醫師你怕血嗎？外科醫師浴血 -- 惡性脂肪肉瘤病人奮戰的敘事醫學

從開始感覺到肚子變大，腫瘤第二次復發只花了一個月的時間。

「我記得你啊！帥哥醫生！」現在連買早餐都不會被叫帥哥了，隱藏在手術帽、隔離衣裡站在阿姨面前還能被認出來。

這位阿姨我是看著他像正常人一樣進醫院的，只是肚子大了點、大得快了點，這是個惡性的後腹腔脂肪肉瘤。身為住院醫師，照顧的病人來來去去，大多只會遇到這麼一次，不會有長期的感情建立，但她是少數能告訴我他記得我的病人，因為上次開刀也被我照顧過，還好是照顧得很好的部分。

住院後短短一個週末已經進展到壓迫腸胃無法進食、壓迫血液回流造成腳腫、壓迫呼吸開始喘起來、壓迫神經合併背到骨盆痛到坐立難安。

於是最好的決定是立即安排手術。

奮戰的勇士是病人

術中發現腫瘤比想像中血流豐沛、沾黏又難止血，像冰山一樣見不到底而且死死的定在後腹壁。跟家屬解釋後，我們只能進行腫瘤減積、拿掉部分腫瘤留下大部分在肚子裡。術後就跟家人談好DNR，腫瘤很嚴重，若發生緊急狀況急救也對實際病情沒有幫助，一群人七八個家屬，大家很好溝通也有很有共識。

術後仍然順利度過腫瘤繼續出血、拔管的關卡，拿掉一點腫瘤後阿姨也確實過了一段比較舒服的住院日子。浴血的是外科醫師，但奮戰的勇士是病人。

本來答應家屬不要對阿姨提切不乾淨的事實，「如果我的腫瘤馬上又大起來就不要再麻煩你們了啦！」查過文獻，這次復發切除率只有三成，再下次復發切除機會甚至只有一成。有時候病人比旁人想的更樂觀，阿姨反過來安慰我們，最讓我佩服的是在她身上我已經看不到原來的混沌、焦慮或慌亂，而是跟家人慢慢地談 DNR。以前也發生過，病人的第六感會預告某些不得不信邪的事實。
